| Heart 2 Heart is a program developed to help strengthen the emotional, spiritual and psychological well-being of patients who suffer complications of such cardiovascular events as heart attack, congestive heart failure, coronary bypass surgery, etc. The program is a support-system team approach including healthcare professionals such as pharmacists, nurses, psychiatrists, cardiologists and educators, dedicated to easing the concerns and improving the outcome of cardiac patients. | |||
